I don't know how well this applies to Arabic manuscripts, they strike me as more careful and organized than many of the western manuscripts, plus they often include a colophon with the name of the scribe and the date of completion, which is absent from many of the western manuscripts, but I remember one of the early English herbals has a two- or three-page section where there is a mismatch between the drawings and the descriptions/labels, and who knows how many times that was recopied before someone noticed that the drawing and the plant description didn't match.
