I don't know how well this applies to Arabic manuscripts, they strike me as more careful and organized than many of the western manuscripts, plus they often include a colophon with the name of the scribe and the date of completion, which is absent from many of the western manuscripts, but I remember one of the early English herbals has a two- or three-page section where there is a mismatch between the drawings and the descriptions/labels, and who knows how many times that was recopied before someone noticed that the drawing and the plant description didn't match.

The lentil image and description seems to stem from Richard Ettinghausen's book Arab Painting. He calls it the islamic form of the plant, meaning that it's extremely abstract and unrecognizable. I haven't been able to read the full passage though since google books is very annoying.
